Yes they can be a pain to fill out, but we have them here so you can print them out at home and fill them out while wearing your coziest pajamas. Just remember to bring them with you on your first visit! (You may not need all of the forms linked to this page!)
If you have recent medical records (x-rays, MRI images) that relate to your reason for seeking out Chiropractic Care, please bring them with you.
